Reflect on elements of writing a comprehensive client assessment and creating a
genogram for the client you selected.
Part 1: Comprehensive Client Family Assessment
Create a comprehensive client assessment for your selected client family that
addresses (without violating HIPAA regulations) the following:
History or present illness
Past psychiatric history
Substance use history
Family psychiatric history
History of abuse and/or trauma
Review of systems
Mental status exam
Part 2: Family Genogram
Develop a genogram for the client family you selected. The genogram should extend
back at least three generations (parents, grandparents, and great grandparents).
Part 1: Comprehensive Family Client Assessment
The family clients are a mother and a son. The mother, Paula, is a 32-year-old African American single mother. The husband was a veteran and died in the Afghanistan war when she was 3 months old pregnant. She currently works as a supermarket attendant in the nearby town and is a Catholic Christian. The son, Jones is a 12-year-old African American male currently in mid-school. Both live together in a one-bedroom rental apartment.
Miss Paula and her son, Jones were referred to the clinic by their primary care provider for therapy. Miss Paula complains of depression, increased stress, increased alcohol consumption, and insomnia.
