The purpose of this assignment is to identify a practice problem idea and an evidence-based intervention to address the practice problem idea. Data management is essential to drive actions and decisions to improve healthcare outcomes. The content will support the formulation of a literature synthesis related to your practice problem idea, which supports professional formulation, communication, and dissemination skills relevant to the DNP-prepared nurse.

Instructions

In order to create flexible options, we are providing you options on this assignment. Concept maps are an effective way to express complex ideas, especially for visual learners. For this assignment, each of the following sections may be presented either as a narrative or concept map:

Readmission rates among patients with heart failure (HF) are a key practice problem that not only affects the well-being of the patients but also healthcare organizations. High readmission rates affect the quality of life and increase the risk of mortality and healthcare cost.  The high rate of readmission is contributed by several factors such as the poor transition to the community and poor discharge planning. In light of this, this paper explores the issue of readmission in HF patients and the efficacy of a multidisciplinary approach as an evidence-based practice that can address this issue.

Practice Problem and Question

Heart failure is among the key leading cause of readmission. Therefore it is critical to address the issue of readmission among patients with HF. Based on the recent statistics, HF affects approximately 6.2 million adults in the United States, which cost the nation approximately $30.7 billion annually, and spending on the HF is expected to increase to about $53 billion by 2030 (Centers for Disease Control and Prevention, 2020). According to Khan et al. (2021), about 1 in 4 HF patients are readmitted within 30 days of being discharged while more than half of the patients are readmitted within 6 months. The researchers indicate more than a quarter of these admissions can be avoided if the right intervention is adopted. Even though there has been optimization of care, HF readmission remains a challenge. As such, it is critical to reduce the number of preventable readmissions because HF leads to significant mortality with about 30%-40% of the patient surviving one year following hospitalization with HF (Nair et al., 2020). The Centers for Medicare & Medicaid Services (CMS) adopted the Hospital………………………..
